docs: all help docs should have a title

Problem:
Autogenerating a title based on the "foo.txt" filename is not great. And
currently, the PDF result has no title because we aren't doing that for
the "print all" codepath.

Solution:
Add a validation check that ensures every doc has a title.
Drop the filename-based autogenerated title.
