The vendor removal is folded in rather than carried as a follow-up commit, so
this one passes validate-vendor on its own: replacing
grpc_middleware.ChainUnaryServer and ChainStreamServer with grpc's own
ChainUnaryInterceptor and StreamInterceptor drops the last use of
github.com/grpc-ecosystem/go-grpc-middleware outside vendor/ on this branch,
and hack/vendor.sh removes it. Upstream's commit carried no vendor changes of
its own, because on master the module was still reachable from other code at
the time, so this commit is not byte-identical to 49ca0d0d03.
