liuhao1024 f9da99aefc fix(plugins): warn loudly when a plugin is in both enabled and disabled lists ...

A plugin listed in both plugins.enabled and plugins.disabled loaded nowhere
with only a DEBUG-level skip (#131980): no startup error, nothing in config
check, and plugins.enabled still listed it, so the surface looked enabled
while the gate silently dropped it.

The disable still wins (documented precedence), but gate_manifest now
detects the contradiction (matching either the path key or the manifest
name, as the disable check itself does) and logs a WARNING naming both
config keys; the placeholder error surfaces the conflict to introspection.

中文说明：修复 #131980 —— plugins.enabled 与 plugins.disabled 同时列出同一
插件时静默不加载（仅 DEBUG 日志）。保持 disabled 优先，但矛盾时升级为
WARNING 并指明两个配置键；占位 error 同步暴露冲突。
