refactor: remove the /initiate-setup slash command
The command, its alias and every surface handler go together: the registry
entry, the Slack /hermes-only pin, the CLI handler, the gateway rewrite and
its canonical-command entry, the TUI builtin (_cmd_initiate_setup goes with
its only reference in _SLASH_BUILTINS) and the slash.exec pending-input pin.
The skill-invocation projection drops its /initiate-setup header in Python
(agent/skill_commands.py's module-top import of agent.initiate_setup_prompt
goes now, before 4.3 deletes that module) and in apps/shared.
run_inbound's handler lookup is back to f"_hm_cmd_{canonical}": no hyphenated
canonical command remains. The three locale keys go from all 17 packs, and
the desktop slash registry is regenerated.
